Overview
This overview outlines the Occupational Health Facility (OHF) functions that are applicable within the context of Animal Handling. It is not intended to show the full range of department services. More information can be obtained by calling OHF Information Phone (310) 825-6771.
Services
OHF is chartered to provide for employee physical care and maintains outpatient clinic services to meet the special needs of employees on the UCLA campus. The clinical staff comprises of highly qualified doctors, physician assistants and nurses. Services include:
- Pre-employment physicals
- In-service physical evaluations
- Diagnosis and treatment of work-related illness and injury
- Return-to-work examinations and physical evaluations
- Immunizations to support health maintenance
Location
OHF is located at 67-120 Center for Health Sciences. This is on the 6th floor, 7th corridor, Room 120. For directions contact OHF.
Hours of Operation
7:30 a.m. - 4:30 p.m. Monday - Friday
OHF is closed on holidays and weekends
After-Hours Care
If injury or illness occurs when OHF is closed, or urgent immediate medical attention is required, treatment can be obtained at the Ronald Reagan UCLA Medical Center Emergency Medicine Center.
Appointments
Appointments for visits should be made whenever possible for routine examination, testing and immunization. Appointments can be made by calling the OHF Department. For Appointments and Appointment cancellations call (310) 825-6771.
Eligibility to Use OHF Services
OHF clients include:
- All employees of the Medical Center
- All employees of the Campus
- Students that hold UCLA employee status
- Medical Health Services Volunteers
To use OHF for employment services, clients must have an appointment scheduled by a UCLA employment organization
To use OHF for work-related injuries, clients must present an authorization form from a Department Supervisor or designee referring the patient to OHF for treatment. Present a valid UCLA Employee Photo ID.
Fees
Most costs for OHF services are accommodated by funding from the Workman's Compensation Program. Costs of Pre-Employment Physicals and some immunizations are borne by the Occupational Health Facility. Detailed information regarding costs and benefits can be obtained by referring to
