Requirements
Please fill out the form, have it signed, and fax it to the Associate Director of the Real Estate Department at (310) 794-2685. If you have any questions regarding the Form, call Steve Forster at (310) 794-2653.

PLEASE NOTE that the individual signing the Department Authorization Form must have delegation of authority over the funding source.
