The Biological Safety Division is an integral part of the UCLA Office of Environment, Health and Safety (EH&S). Our goals are to minimize the health risk of those who are involved in research using biohazardous materials; minimize the risk to the UCLA community and environment; and meet regulatory requirements. Our department is responsible for assisting the UCLA community in implementing University workplace Environment, Health and Safety policies while complying with applicable federal, state and local regulations and guidelines. In addition to providing biosafety programs, the UCLA Biological Safety Division provides the DOD Site Safety Plan.

Medical History Questionnaire (MHQ) 

The MHQ is an annual requirement for all staff and/or students who will handle animals, animal tissue or who have access to the campus vivarium.

Department of Defense (DOD) Site Safety Plan 

The approved DOD Safety Plan is available from the Biosafety Office.

Occupational Health Facility (OHF) 

OHF provides employee physical care and maintains outpatient clinic services to meet the special needs of employees on the UCLA campus.
